support beyond your home

Companionship and social activities

Sometimes the most important support happens outside the home. Our care professionals take time to get to know you – your interests, routines and preferred way of spending time – so that support feels natural and enjoyable.

We can help you to:

  • Go for a walk, visit the park or local places you enjoy

  • Watch a film or visit the cinema

  • Play board games, cards or other hobbies

  • Visit cafés, shops or community venues

  • Build a simple weekly timetable that gives structure and purpose

Where possible, we’ll match you with a care professional who shares similar interests, so your time together feels like real companionship, not just “care”.

Shopping, eating out and meal support

Eating well is essential for your health and wellbeing, but planning, shopping and cooking can sometimes feel overwhelming – especially if you are recovering from illness or have additional needs.

We can support you with:

  • Planning meals and writing shopping lists

  • Going to the supermarket or local shops with you, or on your behalf

  • Carrying shopping and putting it away safely at home

  • Preparing and cooking meals and snacks

  • Supporting you to eat out safely and confidently

Our focus is on nutritious, enjoyable food that fits your tastes, culture and dietary needs.

Support after a hospital stay

Coming home from hospital is often a relief, but it can also be a worrying time. You may feel tired, less confident or unsure how you’ll manage while you recover.

We can provide short-term or longer-term support to help you:

  • Settle back in safely at home

  • Manage personal care, meals and medication during recovery

  • Attend follow-up appointments and check-ups

  • Rebuild your confidence with everyday tasks

  • Gradually return to your usual routines at a pace that feels right

Support can range from a few hours to several weeks or months. If you need ongoing help beyond the initial period, we will complete a further assessment and work with you to design a tailored care package.

Travel training and confidence building

Using public transport or travelling independently can be daunting, particularly if you have learning disabilities, autism, anxiety or mobility difficulties. Our travel training support is designed to build your confidence step by step.

We can:

  • Plan routes with you, including buses, trains or walking

  • Practise journeys together until they feel familiar

  • Support you to understand timetables, tickets and passes

  • Help you develop strategies to manage anxiety when travelling

  • Gradually step back as your confidence grows

The aim is always to help you become as independent as possible, while knowing support is there when you need it.

Community access and social inclusion

Being part of your community is vital for mental health and wellbeing. Our role is to help you stay connected and involved in the things that matter to you.

We can support you to:

  • Visit family, friends and places of worship

  • Attend day services, college or activities

  • Join clubs, groups or hobby sessions

  • Go to health, social care or benefits appointments

  • Explore new interests at your own pace

We’ll work with you to build a weekly plan that balances structure with flexibility, always led by your preferences.

Specialist support for people living with dementia

Dementia affects everyone differently and can be very challenging for both the person and their family. We aim to maintain independence for as long as possible while keeping you safe and comfortable.

Our dementia support beyond the home can include:

  • Support to attend memory clinics, GP or hospital appointments

  • Gentle, familiar activities that reduce anxiety when out and about

  • Walking, light exercise and time outdoors where appropriate

  • Support to stay connected to family, friends and community

  • Working closely with relatives and professionals to keep routines consistent

All support is delivered with patience, reassurance and respect for your life history, culture and preferences.

If you’d like to know more about how we can support you or a loved one beyond the home – whether through social activities, travel training, hospital discharge support or dementia care – please get in touch.

We’re happy to talk through your situation, including options using Direct Payments, Personal Budgets or private arrangements, and help you design support that feels right for you.
